Duane Alan Cook was born on 17 November 1954 in Sacramento, Sacramento County, California.^1,2 He and Kathy Kai Hines were married on 30 December 1973 in Reno, Washoe County, Nevada.³ He and Kathy Kai Hines were divorced in January 1977 in Sacramento County, California.⁴ He died on 31 October 2009 at age 54 in California. Died of Melanoma from a large mole on back.
